How to Get Your Card
Requirements to Obtaining a Medical Cannabis Card in Arizona
Be 18 years of age or older, or have a legal guardian register as your caregiver
Have a valid Arizona driver’s license, Arizona identification card, or passport
Have an Arizona residential address
Experience one of these qualifying conditions:
Agitation of Alzheimer’s Disease
Or a chronic/debilitating condition or treatment that causes:
Severe and Chronic Pain
Severe or Persistent Muscle Spasms
Contact a Medical Cannabis Certification Center to Schedule an Appointment
Note: Certification centers require medical records from the past 12 months that document your qualifying condition. You can obtain these records in one of two ways:
Request these records from your doctor
Some certification centers have an on-site physician who can generate these records for an additional fee (typically $35-$50)
If approved for a medical cannabis card, your card should be mailed to you within approximately 5-10 business days.
What Does it Cost?
Certification centers charge a fee for certification. Please contact the certification centers for prices.
The state charges a $150 fee to process medical cannabis cards (those on SNAP are eligible for a reduced fee of $75)
The state charges a $200 annual fee to process caregiver cards
If you require medical records, the charge is typically $35-$50
To amend, change, or replace a registry identification card, the fee is $10

A: Unfortunately, no. We are not allowed to accept out of state patients.
Q: Do you accept credit cards?
A: We cannot accept credit cards at this time. We do have ATM’s available onsite at all of our locations.
